It is the responsibility of the person making the nomination to provide the following information.

  1. Vita/resume of nominee
  2. News media or administrators to be notified
  3. Three corroborating letters from others in support of nomination sent by your request.  It is recommended that one be from an administrator from the nominee's school.
  • Your nominee will NOT be considered if all documentation is not received before the October 1st deadline. No nomination is complete without the full packet of documentation.
